We will open the levy with the sheriff, serve the documents, mail to the debtor, complete an affidavit, and file it with the sheriff. You will receive a signed and filed copy of the affidavit via email.
 
Please let us know if you would like the original affidavit and we can mail it to you.
 
Note: We maintain a digital copy for our records of all the documents and the filed affidavit for future reference. 
To complete a bank levy package, we will need the following original documents:
Letter of instruction to the sheriff
Original writ (with an additional three copies)​
Check for $40.00 made payable to the sheriff
We will open at Sherrifs' department, serve to bank, mail to respondent, and file proof of service. 
You will receive the proof of service with lefy order number
Original Documents will be mailed back to you, per request
