Serve the documents, mail to the debtor, complete an affidavit and email proof of service to you. You will receive a signed and filed copy of the affidavit via email.

Please let us know if you would like the original affidavit and we can mail it to you.

Note: We maintain a digital copy for our records of all the documents and the filed affidavit for future reference. 

To complete a bank levy package, we will need the following original documents:

• Letter of instruction to the sheriff

• Original writ (with an additional three copies)

• You will receive the proof of service with levy order number

• Original Documents will be mailed back to you, per request
